Family Bridges
As a parent, family member or caregiver of a person with a developmental or intellectual disability, chronic mental illness or physical impairment, managing everyday life is complicated. Each day is filled with managing service providers, arranging transportation or dealing with bureaucracies. And planning for the future often becomes a low priority.
JCFS is here to help families manage day-to-day care of their loved one, as well as help them prepare for the future, ensuring their wishes are carried out. Our staff will assist families and their loved one with a disability in creating a vision for the future.
We take time to get to know your family to help ensure that people with disabilities live a full and happy life.
We will link you to and work with legal, financial, and other professionals, and can act on your behalf when social services decisions are beyond their expertise.
We're partnering with you in planning for the future. If it becomes evident that JCFS is your best option for successor guardian or power of attorney, we are available.

WE WORK WITH FAMILIES TO:
· Provide case management and coordinate services
· Plan for the future and develop a Letter of Intent
· Help with decision-making regarding service providers
· Link to services
· Act as power of attorney or be named as successor guardian, if needed
· Serve as a monitor or second set of eyes
· Assist with day-to-day challenges of chronic mental illness and developmental disability and their impact on the family
Services to Suit Your Needs
We take pride in getting to know each family so that services are tailored to meet their specific needs. Together we will create a Lifebook (LOI) that contains a collaborative history, present day narrative and the future dreams of their loved one. Our services are flexible and can be initiated in part or as a whole at the request of the parent or caregiver. Perhaps you need help with immediate care; you have to move out of state; or you’re worried about what would happen if you would become ill. We understand that life presents constant change, and we are available for short-term and on-going service.
Fees for service are billed hourly and can be established through a trust or private pay.
